Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Nurse Team Leader for a Certified Home Health Agency (CHHA) serving Manhattan and Brooklyn, NY. Qualified candidates will have RN experience within a Certified Home Health Agency (CHHA) as a Coordinator of Care or Field RN, ability to travel to Manhattan or Brooklyn locations for Home Care Visits, a car and driver's license is required for Brooklyn locations.

In this role, the RN Nurse Team Leader will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. The RN Nurse Team Leader will also review patient insurance information; coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Responsible for precepting new nurses and for supervising peers on their field visits.

Responsibilities:

• To create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the home health plan of care in collaboration with the physician.
• To effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of rehospitalization.
• Supports agency established financial goals.
• Demonstrate expert knowledge unique to the practice of community health nursing.
• Monitors and provides leadership and direction to the field nurses under his/her direction.
• Conducts supervisory visits in the field with the nurses
• Serves as preceptor for newly hired nurses upon completion of orientation.
• Overseas nursing documentation compliance for team

Qualifications:

Graduate from accredited School of Nursing - required; BSN - preferred

Minimum of 3 years Homecare field experience – required; Supervisory or leadership experience - preferred

∙ Ability to multitask and prioritize effectively.

∙ Strong clinical skills.

∙ Knowledge of Home Health regulations.

∙ Ability to supervise, mentor, and coach home care nurses.

∙ Computer skills including EMR and Word.

Competitive Salary $106,000 - 125,600 based on Education, Experience, Nursing Certifications and Bilingual Language Abilities (Spanish/Cantonese or Mandarin) $15,000 Sign On Bonus and Generous Benefits Package!
