The Palliative Care Physician provides specialized medical care to patients with serious illnesses in hospital and community settings, collaborating with an interdisciplinary team to develop and implement comprehensive care plans. The role requires relevant medical licensure, board certification or eligibility in Hospice and Palliative Medicine, and adherence to institutional and regulatory standards.
M Health Fairview is posting a position for a 0.6 FTE Palliative care physician who will be working in the University and community hospitals. The Palliative Care Physician will be a member of the palliative care consultation team and will work in collaboration with other physicians, APPs, palliative care social workers and chaplains to provide timely access and care to patients with palliative care needs. They will provide all necessary acute care services to patients at M Health Fairview, provide timely consultations to requesting clinicians in different settings, ensure proper care, diagnosis and treatment as necessary. The scope of practice of each Palliative Care Physician is defined through the Fairview's Credentialing and Privileging Committees. The Palliative Care Physician is responsible for the care of patients by performing thorough examinations, assessments, and diagnostic evaluations and then development of treatment plan. For this 0.6 FTE position, the palliative care physician's hospital workdays will be proportional to their 0.6 FTE and will be on a rotational basis. Weekend call will also be proportional to the 0.6 FTE.
