Make a difference in your community. ECLARO is looking for a Nurses, Registered for our client in Utica, NY .

Responsibilities:

• Provide injections for clinic clients and complete necessary documentation; make medication sheet for each client that gets injections
• Complete clinical documentation that meets compliance standards, medical necessity guidelines, and agency standards as to timeliness and accuracy of documentation
• Perform home/community visits as needed
• For client's on Clozaril:
• Track required monthly lab work on spreadsheet
• Submit lab work into the REMS system
• If clients are overdue, reach out
• Fax lab work to pharmacies if requested
• Ensure EKGs are done as required
• Complete Metabolics on clinic clients and complete necessary documentation; make any referrals and linkages that are needed
• Complete Health screens on clinic clients and complete necessary documentation
• Maintain spreadsheets for injections/health screens
• Triage client calls
• Review client’s notes and brief relative staff
• Answer calls from the Medline and process requests/handle issues with Scripts, side effects, etc.
• Coordinate/advocate care with medical PCP's and specialties:
• Call doctor's offices and lab sites to get lab results, medical notes
• Call hospitals to get reports on the clients that have gone to the ER or are in-patient
• Obtain Prior authorizations, and handle insurance issues regarding medications, (mostly injections)
• Maintain AOT binder
• Assist prescribers with various tasks as needed

Qualifications:

• License and current registration as a Registered Professional Nurse in New York State.
