Organizational Summary

Maternity Care Coalition (MCC) is a community-based nonprofit organization serving Pennsylvania and Delaware with over four decades of commitment to improving maternal and child health and early care and education. MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children 0—3. We achieve this through direct service, advocacy, and research, in collaboration with individuals, families, providers, and communities. Position Summary

The Community Resource Advocate (CRA) is responsible for answering incoming inquiries from community members seeking reproductive, sexual, maternal and child health services, parenting supports, and social support resources. The CRA collects information about needs and connects individuals to the appropriate health service and/or community resource. The CRA also conducts community outreach as needed to promote MCC programs and services. This position requires in-person and remote work. Phone/text/web hours of operation are from 9:00am to 7:00pm. Position is 40 hours/week; non-traditional hours required to handle evening phone/web/text inquiries.

Essential Function

  • Adheres to MCC’s Standard Operating Procedures (SOP) and CES’ policies and procedures to ensure adherence to agency and program standards
  • Responds to incoming phone calls, text and/or website inquiries from community members seeking reproductive, sexual, maternal and child health services, parenting supports, and social support resources
  • Completes a brief screening to collect information about an individual’s needs and determine service eligibility
  • Provides referrals for MCC programs/service, health service, and external provider referrals to community members in response to self-identified needs
  • Maintains accurate records of individual, family, and provider contacts in MCC databases
  • Completes reports noting referrals, provider contacts, family contacts, and outreach activities
  • Represents MCC at outreach events and provider meetings as requested
  • Conducts health promotion activities by providing MCH education, literacy information, distributing period supplies, and STI prevention materials during outreach event
  • Meets with Supervisor regularly for reflective supervision
  • Strengthens own skills through professional development
  • Ensures confidentiality of records, sharing pertinent information only with written or verbal approval (exception for mandated reporting requirements)
  • Routinely use MCC databases (ETO, Sharepoint) and other Microsoft Office programs including Outlook, Teams, Excel, Power Point, and Word.

  • Understanding of MCC’s mission, goals, Theory of Change, and DEIJ objectives
  • Ability to work independently with a high level of energy and contribute as part of a larger team
  • Strong interpersonal skills as demonstrated by compassionate, culturally aware, courteous, cordial, cooperative, interactions with diverse groups of co-workers, community partners, families, and community members
  • Knowledge of the concepts and practices associated with community health issues and social determinants of health, particularly with pregnant people and parenting families served
  • Significant knowledge of and sensitivity to issues facing the communities served
  • Ability to travel in and around MCC’s service region to conduct community outreach
  • Experience in family management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting interactions.
  • Ability to operate a computer and use a variety of common software programs including Microsoft Office and customized databases.
  • Ability to organize tasks in an efficient manner and follow-through with strong attention to detail
  • Ability to complete tasks in a timely manner

Experience, Education, and Licensure

  • High School Diploma with 3 to 4 years' experience in social service or related field or Associate’s Degree
  • 3+ years’ experience providing services in the community
  • Call center, medical setting and customer service experience encourage to apply
  • Knowledge of community, parenting, and health resources
  • Valid driver’s license and driving experience required
