Visiting Angels is passionate about providing exceptional care for seniors at home , and we’re looking for a Care Manager / Community Educator / Social Worker to join our team!
Are you:
✅ A social worker, therapist, or nurse looking to expand your horizons?
✅ Passionate about building relationships with referrers and families in your community?
✅ Skilled in marketing, education, and senior care advocacy ?
🔹 Key Responsibilities:
🔸 Identify & connect with local healthcare providers (hospitals, nursing homes, assisted living, etc.)
🔸 Build & maintain a database of potential referral sources
🔸 Conduct training sessions on dementia and Parkinson’s care
🔸 Represent Visiting Angels at trade shows, conferences & networking events
🔸 Perform client assessments, care planning & coordination
🔸 Maintain & manage CRM systems to track leads
🔹 Qualifications:
✔ LMSW, LBSW preferred ( OT, PT, LPN, RN considered )
✔ Strong presentation & organizational skills
✔ Experience with PowerPoint, Excel, and CRM systems
✔ Passion for elderly care is a MUST!
