Community Advocate

Full Time | Day Shift working 8am - 5pm

** Pay rate at $45,000/year! **

Lutheran Social Services of Central Ohio is currently seeking a Community Advocate for The Health Center at Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the Community Advocate with Lutheran Social Services:

The Community Advocate is responsible for case managing, relocating displaced individuals, and resolving any issue a tenant may have with a landlord. The Community Advocate works with patients who are living with HIV to improve health and wellness by addressing the social determinants of health. The Community Advocate works in a collaborative relationship with the patient and their family, caregivers, support system, other healthcare providers, and community resources using a patient-centered health approach to support the integrated care model. The Community Advocate helps patients identify health and wellness goals and works toward specific objectives to improve health and deliver effective care. The Community Advocate conducts community outreach to a specified target demographic by managing LSS Health Center social media communication including Facebook and Instagram; in addition to LSS Health Center events webpage page.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Assess housing needs for clients that are displaced.
  • Work with landlords to relocate individuals.
  • Maintain and complete progress notes and client level data in medical records system.
  • Provide Advocacy services to individuals who are encountering housing related problems.
  • Evaluate patients' social determinants of health periodically and adjusting as needed to improve outcomes.
  • Ability to work within the community to provide linkages with clients and needed
  • Collaborate with patients to develop health improvement plans centered on personalized evidence-based goals and provide support toward patients' achievement of those goals.
  • Increase patients' ability for self-management and shared decision-making.
  • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being.
  • Contribute to the development and management of high quality, culturally appropriate digital health promotion materials.
  • Participate in community-based outreach events as needed.
  • Perform other duties as assigned.

Requirements for the Community Advocate with Lutheran Social Services:

  • Bachelor's degree in healthcare, social work, public health, health education or related healthcare field preferred; or equivalent experience with Community Health Worker training
  • High School Diploma, required.
  • Experience in social services required.
  • Excellent communication skills
  • Hands on experience in content management
  • Access to reliable insured transportation required. Valid Ohio driver's license, ability to meet LSS insurance standards required.
  • Current CPR and first aid certificate or willingness to be trained.
  • Proficiency in communication technologies (email, cell phone, etc.)
