JOB SUMMARY The Case Manager I reports to the Program Director and is responsible for providing case management and assisting program clients in their activities and in their progress toward achieving daily/weekly/long-term goals, as needed.
ESSENTIAL JOB RESPONSIBILITIES - Complete client service plan, assessment, HMIS data collection, housing plan and help the client to achieve identified goals.
- Provide clients with instruction, as related to specific housing needs.
- Assure clients understand and follow program guidelines and procedures by explaining program expectations and giving each client a copy.
- Complete documentation as needed to include specialized incident reports, crisis intervention and changes in behavior.
- Work alongside with other staff regarding the coordination of client activities and socialization.
- Serve as a resource person with regard to decision-making when challenges may arise.
- Conduct intakes, exits and maintain case notes.
- Linkage with community resources surrounding client needs and housing.
- Serve as backup staff, should staff coverage be needed.
- Observe and address correction of assigned household duties, including any safety issues.
- Attend case conference and case management meetings
- Familiarity with California Tenant Laws, HUD related documentation and the Housing First principle.
- Think and act quickly and efficiently in emergency situations, while using sound judgment.
- Will drive on agency business, as needed.
- Physical presence at the program is required.
ESSENTIAL JOB REQUIREMENTS - A four (4) year degree in Counseling, Psychology, Sociology or related field OR an Associate of Arts degree and a minimum of two (2) years' experience in a residential setting or shelter program OR a high school diploma/GED and a minimum of two (2) years' experience in a residential setting or shelter program.
- Ability to work effectively with program clients, who may be diagnosed with chronic mental illness or experience substance abuse related issues.
- Current certified training in emergency first aid procedures and CPR preferred.
- Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
- Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
- Ability to pass a criminal background check.
- Ability to pass a pre-employment drug screen and TB test.
