MACS and Diversitat have been serving the Geelong community for more than 40 years with a focus on our multicultural communities. As the new entity, Cultura operates across various sites delivering a range of services including residential aged care, home care and aged support services, settlement, training and education, financial counselling and disability support. We also operate a range of social enterprises including The Pulse community radio and various community events including the popular Pako Festa.

The Role:

The Humanitarian Settlement Program (HSP) Case Manager works collaboratively as part of the HSP Team to provide high level case management, information and referral services to HSP clients during their first 18 months in Australia. This role works within program guidelines to empower individuals to feel safe and secure, to promote their wellbeing, and to enable their full participation in society through connection to culture and community.

The HSP Case Manager is responsible for the development and delivery of the Case Management plan, which will be achieved through developing key collaborative relationships with a number of Cultura staff. These include Adult, Community and Further Education (ACFE) trainers, a Housing Worker, the new arrivals Orientation Coordinator, the Employment team, as well as the HSP Program Manager, and Bicultural Workers.

This position is contracted on a permanent full-time basis.

About You:

  • Deliver Case Management Services to tier 1, 2, and 3 clients in accordance with the requirements of the HSP program;
  • Undertake holistic needs assessments, and develop comprehensive and achievable case plans for each family member that work towards their goals;
  • Deliver short term casework, including support, information, advice and referrals;
  • Provide longer term, tailored support to individuals;
  • Arrange information sessions and activities in response to needs identified;
  • Work in collaboration with local services to ensure identified client needs are addressed;
  • Enter all client information, including client centred case plans, case notes, claim requests into required databases, in an accurate and timely manner;
  • Participate in the arrangement and delivery of orientation to community activities for new arrivals;
  • As part of the team, assist with the identification of emerging needs of individuals/communities and contribute to developing effective responses to these needs;
  • Tertiary qualifications in Social Work (Bachelor of Social Work or Bachelor of Psychological Services), or similar field;
  • Experience working with newly arrived communities, including demonstrated understanding of the needs of individuals/communities, and the services and resources available to them.
